Its operations are organized into five distinct divisions: Home Health, Hospice, Home and Community-Based Care, Facility-Based Care, and Healthcare Innovations (HCI). The Home Health division provides skilled nursing care, social work services with a medical focus, and various rehabilitative therapies including physical, occupational, and speech therapy. Through its Hospice segment, the company delivers comprehensive end-of-life support, encompassing pain and symptom management via palliative care, emotional and spiritual guidance, inpatient and temporary respite services, homemaker assistance, nutritional advice, grief counseling for families, and social worker visits. The Home and Community-Based Services segment focuses on daily living support, offering aid with activities like personal grooming, medication reminders, meal preparation and feeding, light household chores, respite care, transportation, and running errands. The Facility-Based Services division caters to individuals with critical medical needs, demanding intensive care and continuous oversight from medical and clinical staff. Patients treated include those with respiratory failure, neurological and cardiac conditions, chronic wounds, kidney ailments, cancer, head and neck trauma, mental health disorders, and musculoskeletal issues. This segment also manages nursing facilities, a family health center, a rural health clinic, physician practices, and provides physical therapy services. Finally, the Healthcare Innovations (HCI) segment specializes in delivering strategic health management solutions to accountable care organizations. By December 31, 2021, LHC Group's operational footprint included 557 home health facilities, 170 hospice sites, 136 community-based service centers, 11 long-term acute care hospitals spread across 12 locations, and 14 Healthcare Innovations facilities. Established in 1994, the company's headquarters are situated in Lafayette, Louisiana.
